Services We Provide
Medication administration, care planning, chronic disease management, preventative health teachings, monitoring vital signs, physician ordered treatments/services.
Activities of Daily Living such as toileting, eating, grooming, transferring, mobility/ambulation, in a supervised positive environment.
Healthy breakfast, lunch, and afternoon snack, nutrition education, special diet, monitoring of food & liquid intake as ordered by physician, registered dietitian consult, CACFP menu.
Range of motion exercises, dance, music, meditation, yoga, and group walks on the Norwottuck Rail Trail and other activities tailored to skills and abilities that meet individual interests.
Group and individual activities, bingo, movies, community outings, games, arts and crafts, prayer group, men’s group, women’s group, haircut, nail care, celebrations, and other activities that promote social and mental stimulation.
Team meetings, advocacy, resource referrals, family/caregiver support, collaboration/liaison with other local agencies to coordinate services.
Door-to-door transportation to and from the program. BakuCare building and vans are accessible to participants in wheelchairs or with limited mobility.
Available to meet the needs of our participants and their families/caregivers.
